This is the speciment of an IMO general declaration form which is based on IMO Convention on Facilitation of International Maritime Traffic. This form is used upon arrival of a ship at a port and includes : the particulars of the vessel, brief particulars of the voyage and a brief description of the cargo. This document along with other attached documents (Cargo Declaration, Crew List, Crew's Effects Declaration, Ship's Stores Declaration, Passenger List, Maritime Declaration of Health) is handed over to the port authorities upon arrival of the vessel.
